If you would like to add a baby's birth date, or a wedding date, just choose YES in the Dropdown Menu. Adding a date adds on $5. (Date will appear in this format: MM/DD/YYYY)

Leave the name/text that you want embroidered in the Note To Seller box at checkout as you want it to appear. (Feel free to send me an Etsy message with any questions!)
